NHS Health Checks

Are you aged 40-75 years old and do not have one of the following conditions:

• Coronary Heart Disease
• Chronic Kidney Disease, which has been classified as stage 3,4 or 5 within the NICE clinical guideline 182 on CKD.
• Diabetes
• Hypertension
• Atrial Fibrillation
• Transient ischemic attack
• Hypercholesterolemia
• Heart Failure
• Peripherial arterial disease
• Stroke

If not you are eligble to a free NHS Health Check.  These can be booked by calling the surgery and booking in with reception for an NHS Health Check.
